My son, now 15 has been on Remicade for 4 years. We began his treatment for bi-lateral anterior uveitis 6 years ago with combination steroid drops, oral and
MTX both pill and injection. Remicade has been the only treatment that allowed him to finally wean off steroids. For the first 2 years on Remicade he was at
5mg/kg initially then 7.5mg/kg every 8 weeks. Then in year 3 he was upped to 10mg/kg every 10/12 wk. And in year 4 he was at 16 week intervals.
Now due to insurance issues, we may have to end Remicade. He still takes MTX 15mg weekly.
My question is, should he flare, what would be your most likely next course of treatment? And, do we wait for +1 or better before treating?
Right now, his vision is 20/25, no correction, but he does have early cataracts that appeared within the first year on pred forte drops. And given his inability to wean oral pred below 20mg daily without flaring, I'm afraid our emergency measures are limited.
